Restricted pages aren't showing up in the index at all, even for users who can see them. Is there a setting to index them with permissions applied?

Check that the source is set to index permissions rather than only public content, and that the crawling account can read the restricted spaces. Without read access, the crawler never sees those pages, so they can't be indexed with their permissions.
